Fix root on iSCSI install issues

We need to fix the remaining issues that make it difficult to install and boot from an iSCSI root device.

The worst of this has been resolved. There are still problems with setting up the network boot, which we should fix eventually, but for the meantime these are amenable to documentation. See http://testcases.qa.ubuntu.com/Install/ServeriSCSIRoot/KVMExample (not documentation, but a potential source of it).
