Aggregate issue failures to streamline reporting

Right now, each issue failure is treated entirely separately. For every fuzz string for every parameter for every payload, an issue is created, which, if failed, gets added to the list of failed issues reported. Group these failed issues together in some logical way, so that only one issue is reported for each test type for each payload.
