Remove backward compat support for swift-internal://

during hte icehouse development cycle we decided that swift-internal:// should be replaced with swift:// and implemented the change. the implementation was backward compatible and quietly accepted swift-internal://. at some point in the future this compat code should be removed.

compat locations are tagged with "TODO(mattf): remove support for OLD_SWIFT_INTERNAL_PREFIX"
