Generate ICS Form 214 from Sahana data

Detailed specification is on the Sahana requirements wiki. The idea is to pull Sahana data into a structured form that is used by US government agencies using the NIMS/ICS system to respond to emergencies. This is a form that reports on unit staff and is filled by unit leaders every operational period (usually a 12 hour time period).
