Convert RCL to CouchApp

We've decided to try converting Reformed Churches Locator to a CouchApp to get peer-to-peer replication by default, and not have to code it as a feature. The new application architecture is introduced at http://caneypuggies.alwaysreformed.com/wiki/RCLCouchAppArchitecture.
