Separate extensions from operators and allow reassigning and selection of extensions

Extensions (VoIP) are currently linked to operators. This feature will separate extensions from operators. If a configuration option is set, operators will be unassigned from an extension when they end work, and be given a choice of available extensions when they begin work again. This will allow for much easier "hot desking" of telephone interviewing operators where there are fixed VoIP telephones
