Upstream OMAP3: clean up patchstack

As part of the upstreaming of the OMAP3 QEMU patches, clean up the patch stack so that all large patches are split up, all patches have sensible names and commit messages, all junk (format changes, debug trace) is dropped and the patch stack is in a logical order. This will make rebases less painful and make it easier to pull out smaller chunks of code to feed upstream.
