Add port to reported hostnames

For convenience, would be nice to always print port number along an instance's hostname/IP. (or at least have a config option for this) When using multi-instance servers it adds to clarity and convenience, and can help avoid unnecesary mistakes or confusions.

Also, it's possible for someone to have multiple instances of same schemas on a single server, but for different purposes (ETL and OLTP being the most obvious)
