Redesign pt-query-digest query reviews

Goal: a well-designed database for storing the parsed representation of the log file, which will work well for other purposes too. Make it possible to store multiple files, multiple servers, etc into the same set of tables. Remove the interaction between reporting and "reviewing" by making it either-or: saving to tables disables the report. Give a sample SQL query to show "new unreviewed queries" instead.
