Make pt-kill log its actions

Add a --log-dsn option to pt-kill. Every query that it kills should be logged to the specified table. The table should contain all of the columns from SHOW PROCESSLIST, so that the entire process entry can be logged. It should also contain server_id, timestamp, and if possible/
