Add 5.6 PERFORMANCE_SCHEMA-style statement digest to slow query log

Extend the slow query log with an optional statement digest field. This value would be calculated in the same way the 5.6 PERFORMANCE_SCHEMA does it, reusing the implementation if possible.
This would help Percona Toolkit, among other things.
