Document Ceph Usage

Ceph should be better documented for usage with both cinder volumes and nova instance storage. http://www.sebastien-han.fr/blog/2012/06/10/introducing-ceph-to-openstack/ provides a great guide, and could be a good reference in the docs.

Many orgs will wish to use Ceph as it becomes more and more production ready. Documenting usage not only for RBD-based volumes, but for actual nova instance storage allows for live-migration etc.
