avoid orphaned accounts and views

currently the parent_id for accounts is optional, so it is possible to create accounts and views without parent (orphaned)

Only top level views (and I do not see accounts as top level) must have no parent_id
Suggestion
create a check box "is top level" (empty per default) and make parent_id mandatory for all non top level accounts

this will avoid a lot of checks and searching
