Remove and block duplicate BDM creation

Various race conditions such as bug 1427060 have existed within Nova for some time that allowed duplicate BDM records to be created when attaching volumes to instances. This blueprint aims to correct this by first cleaning up duplicate BDM records from the DB with a data migration in R before introducing an additional constraint within the block_device_
