Instances should use cow instead of flat files

Right now, instance disks are stored as files in the system and manually partitioned. This leads to very slow load times and a lot of redundant data. Instances should use Copy on Write to conserve space and speed up launch times.

Feedback at the design summit was that LVMs with CoW are inefficient. It is better to use hypervisor-specific CoW images. Changed the spec to reflect that.
Got basic code in. Debugging. Should be ready for review by tonight.
