More accurate instance status

We currently have two issues with instance status updates:
* nova-compute doesn't periodically poll the status of VM's, so if one crashes, it doesn't show (bug 661260)
* nova-compute is solely responsible for updating the status of VMs, so if it dies, nothing updates VM status (bug 661214)
This needs to be improved in Cactus for a more accurate and reliable operation.
