Support post-mortem debugging on test failure

Nose, our previous test runner, supported post-mortem debugging via cli arguments (--pdb and --pdb-failure). Despite there not being a good way to pass command line arguments via testr, it should be possible to enable post-mortem debugging via an environment variable.
