multiple bindings joins

Currently, Joins in the DistributedSAIL are nested loops as in Sesame. This results in a single query sent for each binding in the left part of the join. Modify the join operator to send sets of bindings to endpoints supporting this (e.g. Sesame2). Then e.g. send one query for ten possible bindings
