Replace AccessPolicy

OS::Heat::AccessPolicy has several issues and should be replaced. It was originally added as a placeholder for a proper implementation of the AWS::IAM::Policy capabilities.

AccessPolicy doesn't represent any actual OpenStack resource and it doesn't handle dependencies correctly, as evidenced by this bug: https://bugs.launchpad.net/tripleo/+bug/1286307.

AccessPolicy should be deprecated and replaced, we should collect the use cases in a wiki and come up with the right design.
