Document HOT pseudo-parameters

Provide a built-in way to refer to the stack name in a Heat template (e.g $STACK_NAME).
The name value would be useful in creating user-friendly resource names. It could
alsothen be made accessible inside created VMs (via metadata or user data) for
cases where a server needs to acquire information about other resources in
the stack it's running in (e.g. a list of all servers ).
