Remove upper database abstraction ported from Nova

It's been agreed to remove the abstraction layer above SQLAlchemy and use SQLAlchemy to its full extent within the reference registry server implementation.

This means complete removal of the code in /glance/common/db.py and the "pushing up" of the code in /glance/registry/db/sqlalchemy/* to /glance/registry/db/*
