Generate unique names for lvm snapshots and moutpoints

The freezer-agent generates unique names for lvm-snapshots and relative mount point, unless provided with a specific value.
This enable the concurrent execution of multiple freezer-agent processess with multiple snapshots.
The freezer-agent is also more resistant to errors such as being unable to create/mount snapshots because of already existing snapshots from (possibly) previous runs.
