(Surveillance) Extending Surveillance Rules for Outbreaks

Two fields, a skip count and a skip test, are added to surveillance (validation) rules to provide functions explicitly requested for surveillance.

Two fields will be added to validation/surveillance rules:

* skipcount (an integer) indicates how many periods in the current
  year (before the current period) to skip in generating samples;
* skiptest (an expression) specifies whether the rule should be
   applied at all; when not empty, it is executed just like the left
   and right hand sides except that it is evaluated even if some of the
   data elements are undefined.

We'll add a JEP function EXISTS which is true for non-null values. As
an immediate measure, an OUTBREAK element can be defined for periods
and orgunits where an outbreak has been confirmed and EXISTS on that
element can be used in the skiptest to rule out samples from those
periods and orgunits.

This provides for the top two requests that came out of the CDC
meeting.
