Generic form for all data enties in program-based tracking

Each different activity in the tracking module should be handled by its customized form. To achieve this without having to make explicit form declarations when adding a new activity to the system, the idea is to use already defined requirements as to what data elements a specific activity required registered on it to build a form from a generic solution, so that the one solution will be able to build a suitable form to any activity that will have data entry.
