Enhance common form input elements with jQuery

Common form items used on the DHIS2 mobile site should be enhanced with jQuery to take advantage of newer html5 technology on browsers that support it. Care should be taken to not to change properties or names that would hinder it to work correctly on non-javascript browsers .
