Implementation of activity plan main window

This functionality should be available through Services(top menu)->Individual Records(drop-down)->Tracking(section on the left menu panel)->Activity plan(menu item under Tracking section )

We need a main window where we build a query to fetch persons. The main window should look as follows https://www.dropbox.com/s/kr51qb0xwj5m51f/ActivityPlan.png (colors are not important).

Field description:
Registering unit (list the selected orgunit) - select an organisation unit
Program: - it is a drop-down with all registered programs for the organisation unit, select a program we are interested in
Stage status: "Select stage status" - drop-down with stage statuses, see below for details
From: "Show stages since" - drop-down with pre-defined values for the selected program
To: "Show stages up to" - drop-down with pre-defined values for for the selected program
Area: - it is a bit not clear now, but should be a specific location of patients
Date: - the date field should specify a date (by default today), a stage status is calculated based on a value of this field

Button: - run a query

Comments on Filters/options to filter the list of Persons:
- select stage status:
--- scheduled (yellow), overdue (red), incomplete (purple), ALL
--- default is ALL
