Categorize fields, using titles for various groupings, for ease of entry and display

The current linear formatting for the "add entry" section is hard to use, particularly when it comes to the true/false selections. It would improve the UI dramatically to have questions and options grouped--with headings--into categories (ie- "Sector" "Type of Business/Organization" and "Legal Structure", etc.. In the case of the multiple selections fields, these could be in a two column layout so that they were more condensed.
