Add manage/unmanage support to the LeftHand driver

The LeftHand driver does not currently support manage/unmanage. This functionality can be added now.
The implementation of it can be similar to how the 3PAR FC and iSCSI drivers have implemented manage/unmanage:
During an unmanage rename a volume to "unm-<id>"
During manage rename the volume back to "volume-<id>"
This format allows an admin to easily tell which volumes are currently unmanaged on the LeftHand.
Adding this support will allow admin's using LeftHand to unmanage volumes and then manage them again later.
