Add ConsistencyGroupVolumeTypeMapping table

"volume_type_id" column in "consistencygroups" table contains comma-separated UUIDs of volume types. It should be migrated to a new table ConsistencyGroupVolumeTypeMapping to allow proper representation of the many to many relationship between the "volume_types" table and the "consistencygroups" table.
