Create new attach/detach api's

The current attach/detach flow and use of initialize_connection and terminate_connection has become horribly abused and rather broken. In an effort to simplify things and allow the ability to do things like multi-attach correctly, we should consider revisiting our attach/detach workflow BOTH in Cinder and Nova.
