CDG gui Updates

The CDG gui needs the following:
* a Quit button.
* Separate name and email address text boxes for Creator and Contributors.
* Separate entry boxes for each contributor. Default to 3 boxes and allow an "Add More" button.
* Mark these entries as mandatory:
- title
- date
- description
- creator
- subject
- rights
