Design Mode: Proper Sizing Phase 1

Carve's Design Mode needs true support for relatively-sized documents/elements with and without viewBox attribute. There are several combinations to consider:

a) width/height absolute value (pixels for now) specified, viewBox not specified
b) width/height absolute value specified, viewBox specified
c) width/height not specified (100%/100%), viewBox not specified
d) width/height percentage value specified, viewBox not specified (general case of c. above)
e) width/height percentage value specified, viewBox specified

In all cases, Carve should properly size the canvas and the graphical items correctly
