Handle Extensions Through AppCenter

Currently all apps manage their own extensions. It's very decentralized, can be inconsistent, doesn't provide a method for extension discovery, and is overall a very lacking implementation.
We should manage all extensions automagically and centrally through AppCenter, much how USC handles "helpers" expect let's do a good job at it.
